What to Expect in Your First Therapy Session

Starting therapy can feel awkward before it feels helpful. Many people arrive unsure what to say, how much to share, or whether they are “doing it right.” The good news: a first session is usually gentle, practical, and focused on getting to know you.

The first few minutes are about comfort and consent

Your therapist will likely explain confidentiality, privacy limits, and cancellation policies. This helps set clear expectations.

They may also ask what brought you in. You do not need a polished story. A simple answer is enough:

  • “I’ve been feeling anxious.”

  • “I’m having trouble coping.”

  • “Something happened and I need support.”

  • “I’m not sure, but I know I don’t feel like myself.”

You are allowed to go slowly. A first session is not a test.

You will talk about what is happening now

Most therapists ask about current concerns, past experiences, relationships, work or school stress, health, sleep, and coping habits. In psychotherapy, this helps the therapist understand the bigger picture rather than focusing on one symptom alone.

You can choose what feels safe to share. If a question feels too personal, it is okay to say, “I’m not ready to talk about that yet.”

You may set early goals together

Near the end, the therapist may ask what you hope will change. Goals can be specific or broad. For example, you might want to manage panic, process grief, improve communication, or feel less stuck.

The takeaway

A first therapy session is mainly a starting point. You can expect questions, listening, and a chance to decide whether the therapist feels like the right fit. You do not have to have everything figured out before you begin.
